Bug fixes

• Fixed an issue where new gaps in fill-in-the-gaps questions were inserted in the wrong place
  Previously, in the question editor for fill-in-the-gaps questions, gaps were sometimes added in the wrong place when using New gap or the gap menu above the text field. Gaps are now inserted exactly at the cursor position. This applies to both adding a new gap and restoring a gap from the same menu.
   
• Fixed an issue where duplicate objects were not detected and removed during the synchronisation of assignments and exercises
  Previously, the synchronisation of exercises would not removed duplicates, causing the out of sync notice to remain after syncing. For example, if a question had two criteria with the exact same name and content, the synchronisation would not remove the duplicate if it was not present in the question bank. Now, the synchronisation recognises and removes these duplicates, resulting in fewer unsuccessful synchronisations.
   
• Fixed an issue where question bank assignment attachments were not sorted consistently
  Previously, attachments in question bank assignment settings were not sorted consistently. Now, attachments are sorted alphabetically by file name, matching the ordering used for course assignments.
