Question content is remembered when it is not necessary
Publication date
7th of July 2026
Summary
When creating multiple questions in succession, the content of the previously created question may be retained in the newly created question instead of starting with empty fields.
Steps to reproduce
- Create a new question and enter content.
- Immediately create another new question.
The expected result
The newly created question starts with empty fields and does not contain content from the previous question.
The actual result
The newly created question may contain content from the previously created question. This happens because, in some cases, the application server processes the request faster than the JavaScript can clear the saved client-side state. As a result, the server replaces the relevant elements before the saved state has been cleared. There is currently no workaround other than manually removing the copied content before continuing.
Underlined text causes an error in combination with other styling in written assignments
Publication date
18th of May 2026
Summary
The preview in written assignment breaks if other styling is applied to underlined text. An error is shown and it is not possible to generate forms.
Steps to reproduce
- Create a written assignment and add text to a content block, test instructions, or a question.
- Underline text and add more styling, for example highlighting the text.
The expected result
The combination of underlining and other styling appears in the preview of the written assignment.
The actual result
The preview breaks and the error "! Package soul Error: Reconstruction failed." appears. There is currently no workaround. It is not possible to use styling in underlined text in written assignments.
The plugin we use for inputting numbers does not support preserving the last decimal zero
Publication date
11th of February 2026
Summary
When using the ‘numeric’ gap for a fill-in-the-gaps question, numbers entered in the gap that end with a zero after the decimal point automatically have that zero dropped. We use a plugin for entering numbers (https://docs.autonumeric.org/), and this plugin does not support preserving the last decimal zero.
The steps to reproduce
Create a digital test with a fill-in the gaps question and select 'numeric' as the gap type.
Preview the test.
Enter 12.50, for example
Click 'Start reviewing' to preview the result/answer
The expected result
12.50 is displayed as 12.50.
The actual result and workaround
12.50 is displayed as 12.5 in both the answer sheet and the student’s input field.
Use the ‘alphanumeric’ gap type instead.
Using unsupported background colour in written assignments breaks the preview
Publication date
17th of September 2025
Summary
Unsupported background colours cause the preview of written assignments to not load without showing an error message.
The steps to reproduce
Create a written assignment.
- Use an unsupported background colour in the html.
The expected result
An error message is displayed indicating what is breaking the preview.
The actual result
The preview is not displayed at all.
Shuffling the answer alternatives for multiple-choice questions in a question bank assignment, including other layout options in the question bank assignment settings.
Publication date
16th of September 2025
Summary
The layout settings, such as "Shuffle the answer alternatives for each question" do not work in question bank assignments. When adjusting the settings, it does not reflect in the preview of the question bank assignment. The settings will be visible when the assignment has been copied to the course.
The steps to reproduce
For example for "Shuffle the answer alternatives for each question":
Create a question bank exercise with a multiple-choice question.
Create a question bank assignment and add the exercise to it.
Notice that the answer options are shuffled by default, since the shuffle setting is enabled.
Disable the shuffle setting, but observe that the answer options remain shuffled.
The expected result
The answer alternatives are not shuffled when the setting is disabled, and the preview and edit view are equal.
The actual result
The answer alternatives are still shuffled.
No workaround - this is only the case in a question bank assignment, and does not happen in a course assignment, meaning students are not affected.
The rounding off at numbers above 7 digits with the basic calculator
Publication date
2nd of June 2025
Summary
The basic calculator in a digital test starts rounding off at numbers above 7 digits. The calculator is a third party plug-in and this is something we cannot change, unfortunately.
The steps to reproduce
- Go to the 'Accessibility' settings of a digital test
- Allow participants to use the built-in (basic) calculator
- Take the test
- Do a calculation (e.g., '123 456 789 - 1')
The expected result
The calculated number is precise.
The actual result
The calculated number above 7 digits is rounded off.
Workaround - using the scientific calculator, however, it is rounded above 9 digits for very large numbers.
The checkbox "Start with zero points" incorrectly displays as an option that can be enabled when grading with rubrics or slider
Publication date
13th of May 2025
Summary
The "Start with zero points" option is available for all grading methods when creating a grading scheme for a question. However, this option is only applicable to the Points per Criterion grading type.
The steps to reproduce
- Go to the grading scheme of a question
- Click the more_horiz-icon and click Edit.
- Click the dropdown menu Grading type* and choose Rubrics with Levels or Slider.
- Click the more_vert-icon for more grading options and select Start with zero points.
- Click Save.
The expected result
The "Start with zero points" option should not be displayed when the grading method is switched to Rubrics with Levels or Slider, as it is not relevant to these grading methods.
The actual result
The "Start with zero points" option remains visible even when the grading method is changed to Rubrics with Levels or Slider. If enabled, it does not affect the grading scheme for these methods.
Not possible to use code in multiple choice alternatives in a written assignment
Publication date
14th of April 2025
Summary
When using code in an answer of a multiple choice question in a written assignment, the preview gives an error.
The steps to reproduce
- Create a written assignment.
- Create an exercise.
- Create a multiple choice question.
- Add code in a multiple choice answer.
- Save the question.
The expected result
The code is displayed.
The actual result and workaround
The preview gives an error.
Workaround - creating an image of the code and using the image in a multiple choice answer.
Using the arrow keys when reviewing spreadsheet questions (open beta) navigates to the previous or next question
Publication date
6th of February 2025
Summary
When reviewing a spreadsheet question, using the left and right arrow keys to try to move amongst cells would inadvertently navigate to the previous or next question.
The steps to reproduce
- Create a digital test.
- Add at least two questions, with one being a spreadsheet question.
- Take the test with at least one participant.
- Review the test.
- Try to navigate within the spreadsheet question using the left and right arrow keys.
The expected result
Navigates to the according cell within a spreadsheet question by using the left and right arrow keys.
The actual result and workaround
Navigates to the previous or next question.
Removing an image from the image description type cannot be reverted by clicking on 'Cancel'
Publication date
5th of February 2025
Summary
In the image description type, it is mandatory to upload an image to the description in order to save it. If the image in the image description is removed, the image description cannot be saved. Cancelling the action should discard any changes that were made, meaning that the image should appear again. However, this does not happen, leaving the image description empty.
The steps to reproduce
- Create an exercise in either a question bank or digital test.
- Create an image description type.
- Upload an image.
- Click Save.
- Click on the more_horizicon and Edit.
- Remove the image by clicking on the close icon. Click Confirm to 'Are you sure?'.
- Click on Cancel.
The expected result
Ans should revert all changes made and should show the removed image again.
The actual result and workaround
The removed image is not restored and the image description is empty. Unfortunately, there is no work around at the moment. The image needs to be manually re-uploaded.
Incidents are not created when Monitoring and DocReader are enabled in a digital test
Publication date
20th of September 2023
Summary
While taking a digital test that has the beta feature 'Monitor participants during a digital test' and DocReader enabled, incidents will not be created, allowing students to "potentially" perform tab switches without being detected. Please note that this is an extreme edge case, as students have to click on the test area (left panel) again in order to continue answering their questions, thus resetting the behaviour.
The steps to reproduce
- Create a digital test with DocReader and Monitoring enabled
- In the settings of your test, click on the tab Proctoring and enable Report tab exiting
- Open one browser window where you can see the Taking overview, for accessing the participants’ incidents overview
- Open a second browser window (as student) in digital test, with PDF attachment in split screen mode
- Click on PDF
- Switch tab
- Switch back to digital test tab
- Click on the PDF attachment first, then on the digital test half of the screen
The expected result
This action should be detected as an incident.
The actual result and workaround
Currently, there is no workaround.
Readspeaker does not prevent downloading files via DocReader
Publication date
24th of April 2023
Summary
With the Readspeaker integration enabled, students are still able to download a document after this action has been disabled in the DocReader settings.
The steps to reproduce
- Create a digital test and enable ReadSpeaker in the general test settings
- Add an attachment to the digital test and disable the attachment in the Attachments settings
- Disable downloading a document in the Readspeaker settings on school level
- Disable downloading a document in the Readspeaker settings in the digital test
- Preview the digital test and go to the menu of DocReader
- Click on Download document to download a document
The expected result
The option to download a document should not be available in the DocReader menu since we disabled the possibility to download a document.
The actual result and workaround
It is possible to download an attachment despite this has been disabled in the settings. Unfortunately there is no workaround at this moment.
Objectives insights on test level cannot be calculated correctly when randomised sections are used
Publication date
10th of March 2023
Summary
In the objectives insights of a digital test using randomised sections, the randomisation is not taken into account when calculating the scored values on the objective. This will result in a biased overview of the objectives insights.
The steps to reproduce
- Create a digital test with randomised sections.
- Add at least two exercises to one section. The exercises should be attached to the same objective.
- Set the amount of selected exercises in the section in such a way that the student will not receive all of the exercises of the section in the test during taking.
- Student takes the test.
- Check the objectives insights under the test insights.
The expected result
The objectives insights on test level take sections into account and correctly calculate and display the scored value on the objective.
The actual result and workaround
The objectives insights on test level do not take into account that the participants did not actually take all exercises in the test. We do not know if an objective was present in some of the questions or in all of them. Even if the participants answered all questions, that are attached to the same objective, correctly, the objective insights will still not reach 100%. This does not apply to the objectives insights in the result of one individual participant. On this student level, the insights do calculate and display correctly.
At the moment, no workaround is available for this issue. For this reason, a warning will be added to the objectives insights when the test makes use of randomised sections.
Words are running off the page in written assignment
Publication date
9th of January 2023
Summary
For written assignments, long words at the end of a sentence sometimes run off the page.
The steps to reproduce
- Create a written assignment.
- Create either a description or a question.
- In the text editor, write some text. The line width of a text box is, depending on the used characters, approximately 100 characters. Make sure that the last word on the line starts approximately 10 to 15 characters before the end of the line width. The word should be without any spaces.
- After saving the text, the text will run off the page.
The expected result
Ans should recognise the end of a page and wrap the text on a new line in case the length of the text exceeds the line width.
The actual result and workaround
Ans does not wrap the text and continues the text on the same line, which makes it run off the page. The workaround is to manually add an enter before the long word reaches the end of the line.

It is not possible to add content after a code block in a description
Publication date
9th of January 2023
Solve date
April 27th, 2026
Summary
If the text editor is used, it is no longer possible to add other content after a coding block.
The steps to reproduce
- Create a written assignment.
- Create a question or content in an exercise.
- Type some text in the text editor.
- Use the HTML functionality of Ans to insert the code (make use of the <code> and </code> tags).
- Close the HTML editor and return to the text editor.
- Type your text in the code part.
- Try to move to the end of the code part and try to type text after this part.
The expected result
After skipping to the end of the code block, it is expected that you can continue typing with normal text.
The actual result and workaround
Ans will continue to type in the code block. Ans provides three workarounds.
- You can manually add a regular text below the code part by making use of the HTML editor within the text editor.
- A second workaround which achieves the same result is to first start typing two lines of text, separate them with an enter. Now open the HTML editor and start a coding block in between the two regular text blocks.
- Another workaround would be to use the Code content type for the part where you use coding in your question. This workaround is applicable when you are creating questions only. It is not available for the text editor for students in the open question for example. They would have to use the first possibilities as workarounds.

Publication date
20th of March 2025
Solve date
29th of August - the release of SEB 3.10
Summary
The use of docReader, which gives participants the option to read attachments that are uploaded to a digital test using docReader, does not work well with SEB version 3.8 (Windows).
The steps to reproduce
- Create a digital test.
- Add an exercise with questions.
- Add an attachment (with text to be read aloud with the ReadSpeaker integration).
- Enable the ReadSpeaker integration.
- Enable the docReader option.
- Enable the Safe Exam Browser integration.
- Take the test with SEB 3.8 (Windows)
- Try to open the attachment and select a text to read aloud with the ReadSpeaker.
The expected result
The text in the attachment is read aloud.
The actual result and workaround
The text in the attachment is not read aloud.
Workaround - enforcing the use of version 3.9 (Windows) in the settings of SEB. However, the version 3.9 has issues with order questions (see the open known issue below).
Order questions do not work with Safe Exam Browser (SEB) version 3.9 (Windows)
Publication date
10th of March 2025
Solve date
29th of August - the release of SEB 3.10
Summary
When using the newest version (3.9) on Windows introduced by SEB, it is not possible to drag-and-drop alternatives of order questions.
SEB has confirmed that the drag-and-drop issue will be a configurable setting in the next version of SEB (3.10), which is scheduled to be released on August 29th (link to the milestone).
The steps to reproduce
- Create a digital test.
- Add an exercise with order questions (drag-and-drop).
- Enable the Safe Exam Browser integration.
- Take the test with SEB 3.9 (Windows)
- Try to answer the order questions.
The expected result
The alternatives can be dragged and dropped, allowing to answer the order questions.
The actual result and workaround
The alternatives cannot be dragged and dropped, preventing answering the order questions.
Workaround - enforcing the use of version 3.8 (Windows) in the settings of SEB. However, the version 3.8 has issues with docReader, when the ReadSpeaker integration is used (see the other known issue).
Using the equation keyboard when answering a question will end a Proctorio exam session
Publication date
2nd of May 2025
Solve date
8th of June 2025
Summary
When a student opens the equation keyboard in a question while Proctorio is active, the exam session ends automatically, kicking out the user. Proctorio does not allow the usage of the equation editor within their environment.
The steps to reproduce
- Create a digital test
- Create a question in which the equation editor can be opened (such as an open question or mathematical equation question)
- Enable Proctorio
- Take the test
- Open the equation keyboard
- Click a button on the equation keyboard
- The exam session ends automatically
The expected result
The student can use the equation keyboard to answer their question
The actual result and workaround
The exam session ends automatically, kicking out the user.
Workaround - to allow students access to an external LaTeX editor during the exam (e.g., Overleaf).
Readspeaker does not highlight answer options when reading out multiple choice alternatives
Publication date
25th of February 2025
Solve date
16th of March 2025
Summary
When students are using Readspeaker in a digital test for reading out a multiple choice question, Readspeaker will only highlight the question as it is reading it out. The multiple choice alternatives will not be highlighted as they are being read out aloud.
The steps to reproduce
- Create a digital test.
- Create a multiple choice question.
- Enable Readspeaker.
- Preview the test.
- Click Listen in the top right corner.
The expected result
Readspeaker highlights the entire question including the multiple choice alternatives when reading out the text.
The actual result and workaround
Readspeaker only highlights the question and not the alternatives when reading the question. Students can still select/highlight each alternative and click Listen.
Copying underlined text from Word is not pasted as underlined text in Ans
Publication date
9th of January 2023
Solve date
6th of March 2025
Summary
If you try to copy-paste underlined text from Word to Ans, for example when copying questions from a Word document, underlined text will paste without the underline.
The steps to reproduce
- Write text in a Word document (or another text editor).
- Underline (part of) the text.
- Copy the text.
- Create a field where you can enter text in Ans, for example a question or a description.
- Paste the text.
The expected result
The expected result is that underlined parts of the text are included when pasting the text into Ans.
The actual result and workaround
When the sentences from Word are pasted in Ans, all text formatting is pasted, except for underlining. You will have to manually go over the pasted text and fix the parts of the text which are not underlined manually. If the whole text is underlined, a workaround would be to first enable the text editor with underlined formatting= by making use of the U symbol in the text editor or by using control + u (Windows) or command + u (Mac). After that, paste the text and it will appear underlined.
Problems recognising the 'm' alternative in multiple choice questions with 13+ answer alternatives
Publication date
24th of July 2024
Solve date
5th of January 2025
Summary
In a written assignment, the option 'm' in multiple-choice questions with 13 or more answer alternatives is not always recognised correctly. Typically, the answers of multiple-choice questions with one correct alternative are circles with letters inside. When there are 13 or more answer alternatives, the 13th circle contains the letter 'm', which, when scanned, can be incorrectly recognised as being filled in by the student.
The steps to reproduce
- Create a written assignment.
- Create a multiple-choice question with one correct alternative.
- Add at least 13 answer alternatives.
- Generate forms and print/download the assignment.
- Scan/upload the assignment.
The expected result
Ans should correctly register the selected answer or indicate no answer if none is chosen.
The actual result and workaround
Often, the letter 'm' is incorrectly recognised as being selected, resulting in either:
- Two choices being marked (one of them being 'm'), leading to the question being marked as 'ungraded', or
- The 'm' choice being marked as selected even when no other answer is chosen.
To avoid this issue, we recommend not using more than 12 answer alternatives for multiple-choice questions with one correct answer in written assignments.
