The evaluation form question is a question type that can be added to a hand-in assignment and an appraisal form for reviewers to base their feedback on and award their points to. The work submitted in a hand-in assignment, such as an essay or thesis, can be assessed using the evaluation form question. The evaluation form question can also be used to assess students on various competencies during an oral interview or class presentation. This question type will not be visible to students when they hand in their assignment or during the assessment.
Evaluation form questions will need to be graded manually. To support you in doing this, you can make use of criteria, rubric with levels or a slider.

Create an evaluation form question in a hand-in assignment
To create an evaluation form question in a hand-in assignment, follow the steps below.
- Navigate to your question bank or your course hand-in assignment.
- Click New exercise, fill in a name and click Create.
- Click + New question.
- Formulate the competency on which the student will be assessed in the 'Question*' field.
- Fill in the number of points the question is worth.
- Optionally, add a model answer.
- Select the scoring method:
- Criteria: Grade by selecting each applicable criterion.
- Rubric: Grade by selecting on criterion per set.
- Slider: Grade by manually entering a score.
- If the selected scoring method is 'Criteria' or 'Rubric', add the Evaluation criteria. This option is not relevant for the scoring method 'Slider'. Click on Add criteria to add a criterion. Optionally, click Add group, to add a group. More information can be found here.
- Optionally, add learning objectives in the 'Objectives' field.
- Click Update.

- Click + New question.

Add a grading scheme to your evaluation form question
When adding an evaluation form question to a hand-in assignment or an appraisal form, both the exercise and the grading scheme are added in the Grading scheme in the menu at the top. The steps on how to add a grading scheme to an evaluation form question are explained above.

Options for an evaluation form question
For additional options, click the edit-icon. Then click on the settings-icon. You will see the following:

Bonus
This marks the question as a bonus question, which means that its points are not included in the maximum amount of points achievable for that assignment. If a student answers the question incorrectly, this will not be detracted from the number of points and the mark. However, when awarding points, it is possible to achieve more points that the maximum points achievable for the assignment.

Partial scoring
Students will receive points when their answer is partly correct. For example, if the total number of points they can receive for a question is two and their answer is 50% correct, they will still receive one point. Without this option, they can only receive zero or the maximum number of points.
