Bug fixes

• Fixed issue where it was not possible to add images or files to the text instructions of question bank assignments
  Previously, when adding images and/or files to a description field of a question bank assignment's test instructions a loading bar would appear and the image/file would not appear in the description. This has now been fixed. 
• Fixed an issue where exporting marks to an LMS would stop if it encountered an error.
  Previously when exporting marks to an LMS, the export would stop if sending the mark failed and skip all the remaining marks. Now the remaining marks will continue exporting and a log message will be printed for the failed requests.
• Improved the scan page performance
  We improved the performance of the scans page by adding an index as the page would previously timeout for some users.
